Blood Tests – Direct Laboratory Services (DLS)
Serious medical conditions such as Heart Disease, Prostate Cancer, Diabetes,
Thyroid Disease, and more, can go undetected for up to two years without noticeable
symptoms. Comprehensive Blood work performed in a timely manner could help prevention
and/or early detection of diseases. The earlier a problem is detected the easier

by providing a simple, low cost, high-quality screening program directly and confidentially
to its members. The program does not require a Doctor’s appointment or lab order.
Members can get their blood tests done with savings of up to 70% off typical laboratory
costs. The tests available are the same lab tests offered by Physicians, and are
analyzed by the same Clinical Laboratory Improvement Amendments (CLIA) – certified,
accredited labs such as LabCorp. Moreover, the test results are reviewed and optionally
interpreted by licensed Physicians on staff. Affordable Healthcare Plan members
save the cost of the doctor’s visit to order the lab work. It is just the cost
of a Doctor’s visit and lab order that is skipped, the Physician is not, however,
left out of the loop.
The Comprehensive Wellness Profile (CWP) blood lab work offered by with your
AMICUS Program includes blood panels* and profiles* on all major organs of your

How to use this program:
1. To order a CWP blood test and locate the Patient Service Center (PSC) blood
draw site of the major Clinical Laboratory nearest you, call 1.800.908.0000, Monday
through Friday, 8:00 am – 5:00 pm, CST, and press “0” for a representative.
2. Have your membership card handy, as you will be asked for your member ID number.
The CWP blood test is $89 for each AMICUS member, which amounts to ~ 70% savings
off the $300+ retail value of the test.
3. The $89 test charge can be paid either by a major credit card or with check
over the phone.
4. A copy of your order and requisition for blood draw will be mailed and delivered
to you in 3-5 days.
5. Most of the PSC blood draw sites allow and use Walk-In testing. Make sure
you take the paperwork and your membership card with you, otherwise you will be
billed the much-higher “Retail Patient Price” of $300+.
6. Your blood tests results are generally released and posted within 48-72 hours
from the time blood is drawn, and a hard copy is mailed to the patient the same
day. The test results are reviewed by a resident Physician (Health Consultant),
and any out-of-range results are pointed out.
7. A more thorough interpretation of your test results can be obtained through
a telephone consultation (extension 204) with the Health Consultant. There is
a fee for this optional service.

order; however when you are scheduled for a physical or other diagnostic visit,

an office visit to get test results to discuss with your physician. |
